Pantoprazole 40mg + Domperidone 30mg combines a proton-pump inhibitor with a prokinetic medicine for selected reflux symptoms associated with nausea or impaired upper-gut motility. It addresses acid-related irritation and movement of stomach contents through different mechanisms.
How to use
Take before a meal, usually about 30 minutes before breakfast. Swallow the capsule whole without crushing the gastro-resistant or sustained-release contents. The domperidone component makes this different from routine long-term pantoprazole-only treatment.
How it works
Pantoprazole blocks the gastric proton pump and reduces acid secretion. Domperidone blocks peripheral dopamine receptors, promoting upper gastrointestinal motility and reducing nausea.
Side effects
Headache, diarrhoea, abdominal discomfort, dry mouth and raised prolactin-related symptoms can occur. Fainting, palpitations or an irregular heartbeat requires urgent assessment.
Important information
Domperidone can prolong the QT interval, especially with electrolyte abnormalities, heart disease or interacting medicines. Strong CYP3A4 inhibitors can increase risk. Difficulty swallowing, gastrointestinal bleeding or unexplained weight loss needs investigation rather than symptom suppression alone.

Pantoprazole suppresses acid production over time; it is not an instant neutralising antacid.
Why take it before breakfast?
Pre-meal dosing helps pantoprazole act when meal-stimulated acid pumps become active.
Can clarithromycin or an azole antifungal interact?
Some of these medicines raise domperidone exposure or prolong QT, increasing arrhythmia risk.
Does every reflux patient need domperidone?
No. The prokinetic component is useful only when its additional role fits the patient's symptoms and risk profile.
