Esomeprazole 40mg + Domperidone 30mg is an acid-suppression and prokinetic combination used for selected adults with gastro-oesophageal reflux disease when esomeprazole alone does not adequately control symptoms. It combines reduced acid production with improved movement of the upper digestive tract.
How to use
Swallow the capsule whole with water on an empty stomach, usually 30 to 60 minutes before the first meal. Do not crush or chew the gastro-resistant and sustained-release pellets. Continued need for the domperidone component should be reassessed rather than treating the combination as indefinite maintenance.
How it works
Esomeprazole blocks the stomach's acid-secreting proton pump. Domperidone blocks peripheral dopamine D2 receptors and promotes upper gastrointestinal movement, helping food pass through the stomach.
Side effects
Dry mouth, headache, abdominal discomfort or diarrhoea can occur. Palpitations, fainting or an irregular heartbeat may indicate a serious rhythm disturbance and require urgent assessment.
Important information
Tell your doctor about prolonged QT, heart failure, low potassium or magnesium, liver disease, gastrointestinal bleeding or obstruction. Clarithromycin, erythromycin, azole antifungals and other QT-prolonging medicines can increase domperidone-related rhythm risk.
Uses
- Selected adult gastro-oesophageal reflux disease with inadequate response to esomeprazole alone.
Benefits
May improve reflux-related discomfort where both acid suppression and a prokinetic effect are useful. It is not automatically a better choice than esomeprazole alone for uncomplicated heartburn.
Why add domperidone to esomeprazole?
The additional ingredient addresses upper digestive motility rather than further blocking acid. It may help selected reflux symptoms when a proton pump inhibitor alone has not been sufficient.
Is this the same as plain esomeprazole for long-term use?
No. Domperidone adds its own adverse-effect and interaction profile. A continuing acid-suppression requirement does not automatically mean the combination is needed for the same duration.
Can domperidone affect breast discharge or menstrual periods?
It can increase prolactin and may cause breast tenderness, milk production unrelated to breastfeeding or menstrual changes. These effects should be reported if they occur.
What should I do if I develop palpitations or fainting?
Stop taking the combination and seek urgent medical assessment. Domperidone can alter the heart's electrical rhythm, particularly with interacting medicines or abnormal electrolytes.
Does this combination treat a blocked stomach or bowel?
No. Increasing gut movement can be harmful when there is mechanical obstruction, perforation or gastrointestinal bleeding. Severe persistent abdominal pain or vomiting needs assessment.
