Pemetrexed is an antifolate chemotherapy used in selected non-squamous non-small-cell lung cancer and malignant pleural mesothelioma regimens. The tumour type and treatment combination determine its role.
How to use
The oncology team prepares and gives the medicine as an intravenous infusion over 10 minutes. Folic acid is started seven days before the first infusion; intramuscular vitamin B12 and a corticosteroid regimen are also arranged to reduce treatment toxicity.
How it works
Inhibition of folate-dependent enzymes limits synthesis of DNA and RNA precursors needed by proliferating cells.
Side effects
Fatigue, nausea, appetite loss, mouth sores, rash and reduced blood-cell counts can occur.
Important information
Blood counts and renal function must be assessed before cycles. Fever, infection, bleeding or severe diarrhoea needs prompt review; impaired renal clearance substantially increases toxicity.

Why is folic acid prescribed with chemotherapy?
It reduces the severity of pemetrexed toxicity and is not an optional general supplement.
Can oral B12 replace the planned injection?
Do not substitute independently; the labelled toxicity-reduction regimen uses intramuscular B12.
Why might a steroid be prescribed around each cycle?
Dexamethasone is used in the treatment protocol to reduce skin reactions.
What fever needs urgent oncology advice?
Any fever during a period of low blood counts should be assessed promptly under the clinic's emergency plan.
Can I take ibuprofen without mentioning it?
NSAIDs can affect pemetrexed handling, particularly with reduced kidney function; timing may need adjustment.
Does every lung-cancer subtype use pemetrexed?
No. Histology, including squamous versus non-squamous disease, affects suitability.
Can a cycle proceed despite abnormal kidney tests?
The oncology team must evaluate renal function before deciding on treatment.
Can I change the supplement schedule after the last infusion?
Continue folic acid throughout treatment and for 21 days after the last pemetrexed dose. Do not stop it on the final infusion day.
