Calcium folinate 15mg, also called leucovorin, is a reduced folate used to limit toxicity from methotrexate and certain other folate antagonists. It is a timed rescue treatment, not a calcium supplement or an ordinary multivitamin.
How to use
Take each rescue dose at the exact time specified in the methotrexate treatment plan, with or without food. The start time and subsequent intervals depend on that protocol. Contact the oncology team immediately if a rescue dose is missed or vomiting prevents absorption.
How it works
Folinic acid supplies a form of folate that can enter cellular folate pathways without the blocked dihydrofolate-reductase step. This helps restore folate-dependent processes in vulnerable normal cells.
Side effects
Oral folinate is often well tolerated, but allergic rash, hives and rare serious hypersensitivity can occur. New symptoms during chemotherapy may have several causes and should be assessed by the treatment team.
Important information
Rescue dose and duration depend on methotrexate exposure, blood levels and kidney function. Oral absorption becomes less reliable at larger doses. Folinate can mask vitamin B12-related anaemia while neurological injury continues, so it is not a substitute for B12 treatment.

Is calcium folinate the same as folic acid?
No. Folinate is already a reduced folate and has a distinct rescue role. Do not replace it with an over-the-counter folic acid tablet.
Should every patient start rescue exactly 24 hours later?
No. Some protocols use that interval, but it is not universal. Follow the written timing for the actual methotrexate regimen.
Can many tablets replace a large intravenous rescue dose?
Not reliably. Oral absorption is saturable, particularly above 25mg. High-dose rescue or poor absorption may require a parenteral preparation.
Does folinate directly remove methotrexate from the blood?
No. It protects folate-dependent cellular functions. Delayed methotrexate clearance also requires appropriate hydration, monitoring and sometimes other specialist treatment.
Can it change seizure control?
It may reduce the effects of medicines such as phenytoin, phenobarbital or primidone. Tell the oncology team about epilepsy treatment before rescue begins.
Is folinate always protective when combined with fluorouracil?
No. In fluorouracil regimens it enhances anticancer activity and can increase toxicity. That use has a different purpose and protocol from methotrexate rescue.
Can it treat pernicious anaemia instead of B12?
No. Blood counts may improve while B12-related nerve damage progresses. The deficient vitamin must be identified and replaced.
What if I cannot keep a rescue tablet down?
Contact the treatment team without waiting for the next scheduled dose. A change of route may be needed to maintain effective rescue.
