Lenalidomide is an immunomodulatory anticancer medicine used in selected multiple-myeloma, myelodysplastic-syndrome and lymphoma regimens. Indication, blood counts and renal function determine the treatment cycle and dose.
How to use
Swallow capsules intact at the scheduled times, with or without food. Do not open them. Follow the oncology cycle, including planned rest days, rather than taking continuously unless that is the prescribed regimen.
How it works
Binding to cereblon alters protein degradation and immune signalling, contributing to tumour-cell suppression and immunomodulatory effects.
Side effects
Fatigue, rash, diarrhoea or constipation and reduced white cells, platelets or haemoglobin can occur.
Important information
Strict pregnancy-prevention measures apply to women and men because lenalidomide is teratogenic. Blood counts, renal function and thromboembolism risk require monitoring.

Benefits
Can help control multiple myeloma and selected lymphomas. In certain myelodysplastic syndromes it can improve red-cell production and reduce transfusion needs; these uses have different treatment schedules.
Can pregnancy precautions be skipped during a treatment break?
No. Women who can become pregnant must maintain effective pregnancy prevention for four weeks before treatment, throughout treatment and interruptions, and for four weeks after the final dose. Pregnancy testing remains part of the prevention programme.
Do men also need precautions?
Yes. Men must use a latex or synthetic condom during sex with a pregnant partner or a partner who could become pregnant, including during treatment breaks and for four weeks after stopping, even after vasectomy. Sperm donation is not permitted during this period.
Why are blood counts checked frequently?
Bone-marrow suppression can increase infection, anaemia and bleeding risks.
Can kidney disease change the dose?
Yes. Renal clearance is important in determining exposure.
Why might an anticoagulant be prescribed?
Some regimens increase clot risk, so the oncology team may prescribe prophylaxis.
What symptoms could indicate a clot?
One-sided leg swelling, sudden chest pain or breathlessness needs urgent assessment.
What should I do if a capsule breaks?
Do not open, crush or handle the capsule powder. If it touches the skin, wash the area thoroughly with soap and water; rinse exposed eyes with plenty of water and contact the treating team. Pregnant people must avoid handling broken capsules.
Can the 10mg and 25mg capsules be exchanged without changing the prescription?
Only under a revised prescription that accounts for the exact cycle and required daily dose.
