Naltrexone Hydrochloride 8mg + Bupropion Hydrochloride 90mg ER Tablets are used with a reduced-calorie diet and increased physical activity for chronic weight management in eligible adults with obesity, or overweight adults with a weight-related condition.
How to use
Swallow extended-release tablets whole; do not cut, crush or chew them. The regimen is increased gradually to improve tolerability. Avoid high-fat meals because they increase drug exposure and seizure risk. Do not take another bupropion product or an opioid alongside this combination.
How it works
Bupropion and naltrexone act on hypothalamic appetite regulation and reward pathways involved in eating behavior and cravings.
Side effects
Possible effects include nausea, constipation, headache, vomiting, dizziness, dry mouth, insomnia, anxiety, and raised blood pressure. Serious concerns include seizures, suicidal thoughts, severe hypertension, allergic reaction, angle-closure glaucoma, liver injury, and opioid withdrawal or overdose risk.
Important information
Do not use during pregnancy, with uncontrolled hypertension, seizure disorders, anorexia or bulimia, chronic opioid use, acute opioid withdrawal, another bupropion product, or an MAO inhibitor within the previous 14 days. Abrupt withdrawal from alcohol, benzodiazepines, barbiturates or antiepileptic medicines increases seizure risk. Tell your doctor about bipolar disorder, suicidal thoughts, glaucoma, kidney or liver disease, and diabetes medicines. Monitor blood pressure, pulse and mood.

Why is titration needed?
Gradual titration helps reduce nausea and improves tolerability while reaching the target regimen.
Who should not use it?
It must not be used with a seizure disorder, anorexia or bulimia, uncontrolled hypertension, chronic opioid treatment or acute opioid withdrawal. Concurrent bupropion, recent MAO-inhibitor treatment and abrupt withdrawal of certain sedatives or alcohol are also contraindications.
Why monitor mood?
Bupropion-containing medicines can affect mood and suicidal thoughts, especially after starting or dose changes.
Can opioids be taken with it?
No routine opioid use should be combined because naltrexone blocks opioids and can trigger withdrawal or complicate pain treatment.
What if weight does not improve?
Response is assessed after 12 weeks at the maintenance dose. Treatment is stopped if at least 5% of starting body weight has not been lost, because continued use is unlikely to give meaningful sustained benefit. This is 12 maintenance weeks, not 12 weeks including initial titration.
