Pirfenidone 400mg is an antifibrotic medicine used for idiopathic pulmonary fibrosis. It helps slow progression of lung scarring and the associated decline in lung function; it does not remove existing scar tissue.
How to use
Take with food to improve tolerability. Treatment is introduced gradually rather than starting at the full maintenance amount. A substantial interruption may require re-titration. Liver tests are performed before and during treatment.
How it works
Pirfenidone has antifibrotic and anti-inflammatory effects, reducing signalling and cellular activity involved in excess collagen production and fibrotic tissue formation.
Side effects
Nausea, reduced appetite, indigestion, diarrhoea, fatigue, rash and photosensitivity can occur. Jaundice, dark urine or a severe skin reaction requires prompt assessment.
Important information
Use protective clothing and high-factor sunscreen because sunlight can trigger significant skin reactions. Smoking can reduce pirfenidone exposure. Fluvoxamine and ciprofloxacin can increase levels. Kidney or liver impairment can affect suitability.

A sufficiently long interruption can require another gradual increase to reduce adverse effects; the restart depends on the length of the gap.
