Perampanel 4mg is an anti-seizure medicine used for focal-onset seizures and, as adjunctive treatment in appropriate patients, primary generalised tonic-clonic seizures. It reduces excitatory signalling involved in seizure spread.
How to use
Take once daily at bedtime, with or without food. Dose increases are gradual and spaced apart because perampanel remains in the body for a long time. Do not add extra doses after a seizure or stop abruptly.
How it works
Perampanel is a non-competitive AMPA-receptor antagonist. Blocking this glutamate receptor reduces excitatory transmission between neurons.
Side effects
Dizziness, sleepiness, unsteadiness, fatigue and irritability can occur. Aggression, threatening behaviour, severe mood change or suicidal thoughts requires immediate help.
Important information
Behavioural reactions can occur even without a previous psychiatric diagnosis. Alcohol can markedly worsen mood, coordination and judgement. Enzyme-inducing anti-seizure medicines alter exposure. Falls risk is higher in older adults and during dose escalation.

Why is Perampanel 4mg taken at bedtime?
Drowsiness, dizziness and coordination problems are common; bedtime administration can reduce their impact during waking activities.
Why are dose increases spaced out?
Its long half-life means a dose change can take time to show its full effect and tolerability.
Can personality or aggression change?
Yes. New hostility, aggression or marked irritability is a recognised serious concern and should not be dismissed.
Can carbamazepine or phenytoin change its effect?
These enzyme-inducing medicines can lower perampanel concentrations; starting or stopping them changes the treatment balance.
Can alcohol be used normally?
Alcohol can amplify impairment and adverse behavioural effects, making the combination particularly risky.
