Linezolid 600mg/300mL is an oxazolidinone antibiotic infusion for selected serious Gram-positive bacterial infections. Its uses include susceptible pneumonia, skin and soft-tissue infections and vancomycin-resistant enterococcal infection.
How to use
The healthcare team administers the ready-to-use solution intravenously over 30 to 120 minutes. It is not a rapid intravenous push. Do not add other medicines to the infusion container; shared intravenous lines need compatibility checks and appropriate flushing.
How it works
Linezolid binds bacterial 23S ribosomal RNA in the 50S subunit. It prevents formation of the functional initiation complex needed to begin bacterial protein synthesis.
Side effects
Diarrhoea, nausea, headache and vomiting can occur. Low blood-cell counts, visual or peripheral nerve problems, lactic acidosis and serotonin syndrome are important potential adverse effects.
Important information
Blood counts should be monitored weekly, especially during longer courses or with pre-existing marrow suppression. Tell the team about antidepressants, serotonergic opioids, MAO inhibitors and diabetes medicines. Persistent or bloody diarrhoea needs assessment for antibiotic-associated colitis.

- Selected susceptible Gram-positive pneumonia and skin infections. Vancomycin-resistant Enterococcus faecium infection, including associated bloodstream infection.

Does this infusion cover Gram-negative bacteria?
No. Linezolid is not indicated for Gram-negative infection. A separate active antibiotic may be needed if such organisms are suspected or confirmed.
Is 600mg/300mL equivalent to 2mg/mL?
Yes. The total amount is 600mg in 300mL of solution. The volume is not an additional active ingredient.
Can the infusion later be changed to oral linezolid?
Often, oral treatment is possible because linezolid has high oral bioavailability. The infection team decides when swallowing, absorption and clinical condition permit the switch.
Do I need to stop an antidepressant myself?
No. The team must weigh infection urgency against interaction risk and manage any interruption. Do not independently stop psychiatric treatment.
Are aged cheese and fermented foods relevant?
Avoid large quantities of tyramine-rich foods such as mature cheese, fermented meats and yeast extracts during treatment. Linezolid inhibits MAO activity, so excessive tyramine can cause a marked blood-pressure rise.
What should new blurred vision mean during treatment?
Report it promptly for an eye assessment. Optic neuropathy has been reported, particularly with prolonged exposure, and should not be ignored.
Can it affect blood glucose?
Low blood sugar has been reported in people with diabetes receiving insulin or oral medicines. Glucose monitoring may need to be intensified.
Can a course be extended beyond four weeks without review?
No. Longer exposure raises concerns including neuropathy and marrow toxicity. Extension needs specialist assessment and appropriate monitoring.
