Donepezil 5mg + Memantine 5mg combines two medicines used to manage cognitive and functional symptoms of Alzheimer's dementia. The components act on different brain-signalling systems and may be used together when combination treatment is appropriate. It does not cure dementia or restore all memory lost to the disease.
How to use
Take the tablet by mouth at the same time each day, with or without food. A caregiver can help prevent missed or repeated doses. The selected tablet must fit the person's established donepezil and memantine doses; the two components may need separate adjustment during initiation or after a treatment gap.
How it works
Donepezil reduces acetylcholine breakdown by inhibiting acetylcholinesterase. Memantine blocks excessive activation of NMDA receptors by glutamate, helping regulate signalling involved in cognition without being an acetylcholine replacement.
Side effects
Nausea, diarrhoea or constipation, loss of appetite, headache, dizziness and sleep disturbance may occur. Fainting, a very slow pulse, seizures, gastrointestinal bleeding or a major change in confusion needs prompt assessment.
Important information
Tell your doctor about kidney impairment, slow heartbeat or heart block, ulcers, seizures and urinary problems. Kidney function affects memantine dosing. Amantadine, ketamine, dextromethorphan, urine-alkalinising medicines and other drugs that slow the heart can interact. Track appetite, weight, falls and functional changes during treatment.

How does this differ from donepezil alone?
It adds memantine, which acts on NMDA signalling rather than acetylcholine breakdown. It is not simply a higher-strength donepezil tablet.
Can the two strengths be increased together automatically?
No. Donepezil and memantine have different titration and tolerability considerations. A fixed-dose tablet may not provide the dose balance needed at every stage of treatment.
What if several days of Donepezil 5mg + Memantine 5mg have been missed?
Contact the treating clinician before resuming the previous regimen. Memantine may need retitration after an interruption, and a gap of a week or more also matters for donepezil.
Can this combination be used for ordinary forgetfulness?
It is intended for a diagnosed dementia treatment plan. Occasional forgetfulness, poor sleep, depression, thyroid disease and B12 deficiency need a different assessment rather than an automatic dementia medicine.
