Divalproex sodium 1000mg extended-release tablets are used for selected seizure disorders, acute manic or mixed episodes in bipolar disorder and migraine prevention. The prolonged-release formulation supports a scheduled maintenance regimen, not immediate relief of a migraine attack.
How to use
Swallow the extended-release tablet whole at the scheduled time each day. Do not crush or chew it. It may be taken with food if stomach upset occurs. Delayed-release and extended-release products need a prescribed conversion rather than a tablet-for-tablet switch.
How it works
Divalproex is converted to valproate. Its effects include enhancement of inhibitory GABA activity and modulation of neuronal ion channels, helping reduce excessive brain electrical activity.
Side effects
Nausea, tremor, sleepiness, weight gain and hair thinning can occur. Severe abdominal pain, jaundice, unusual bleeding or marked confusion needs urgent assessment for pancreatitis, liver injury, low platelets or high ammonia.
Important information
Significant liver disease, urea-cycle disorders and certain POLG-related mitochondrial disorders are contraindications. Valproate can cause major fetal harm. Migraine prevention is contraindicated during pregnancy and without effective contraception in women who can become pregnant. Epilepsy or bipolar use requires specialist risk assessment. Men planning to father a child should discuss alternatives and contraception before treatment changes; As a precaution, men and their partners should use effective contraception during treatment and for three months after stopping.

Does ER mean I can break the tablet to make a smaller dose?
No. Breaking, crushing or chewing can disrupt the intended release. Use a prescribed suitable strength or formulation for smaller dose adjustments.

Abrupt withdrawal can trigger severe recurrent seizures. Any reduction should follow an individual plan from the treating clinician.
Why are liver tests and blood counts needed?
They help detect liver effects and reduced platelet counts, including changes that may precede obvious symptoms. Monitoring is especially important early in treatment.
What should someone do if pregnancy occurs?
Contact the specialist urgently for a treatment review. Do not stop epilepsy treatment abruptly, because uncontrolled seizures can also be dangerous.
Can meropenem or another carbapenem affect seizure control?
Yes. Carbapenems can sharply reduce valproate levels and cause loss of seizure control. The infection and neurology teams should coordinate an alternative plan.
Why must lamotrigine use be mentioned?
Valproate slows lamotrigine elimination, increasing exposure and serious-rash risk. Lamotrigine dosing usually needs a different titration schedule.
Can ammonia be high even when liver tests are normal?
Yes. New lethargy, vomiting or confusion can indicate hyperammonaemia. Risk is particularly relevant with topiramate and needs prompt evaluation.
What if a dose is forgotten?
Take it when remembered unless the next dose is nearly due; otherwise skip the missed dose. Do not double the next dose, and seek advice if seizures recur.
