Carvedilol 40mg is an extended-release alpha- and beta-blocker used for high blood pressure, chronic heart failure and selected patients with reduced heart pumping function after a heart attack. The prolonged-release formulation supports once-daily treatment as part of ongoing cardiovascular care.
How to use
Take the extended-release tablet once daily with or immediately after food, at a consistent time. Swallow it whole; do not crush, chew or split it. A change from conventional carvedilol tablets requires a planned dose conversion, not a milligram-for-milligram substitution.
How it works
Carvedilol blocks beta receptors in the heart and alpha-1 receptors in blood vessels. It slows the heart, reduces its workload and widens blood vessels. These actions lower blood pressure and, over time, help support a failing heart.
Side effects
Dizziness, tiredness, slow pulse, low blood pressure, diarrhoea and weight gain may occur. Fainting, wheezing, a markedly slow heartbeat or rapidly worsening breathlessness and swelling need prompt medical attention.
Important information
Do not stop carvedilol abruptly because angina and heart problems can worsen. Tell your doctor about asthma, heart block, severe liver disease, circulation problems or diabetes. Verapamil, diltiazem, digoxin and some antiarrhythmics can further slow the pulse. Blood pressure, pulse, body weight and fluid symptoms are followed during heart-failure dose increases.

Why can dizziness be worse when treatment first starts?
Blood pressure may fall when standing, particularly after starting or increasing carvedilol. Rise slowly from sitting or lying down; persistent dizziness or fainting may mean the treatment needs adjustment.
Can carvedilol hide a low blood-sugar episode?
Yes. It can blunt the fast heartbeat that usually warns of hypoglycaemia and may prolong an episode. People using insulin or sulfonylureas need glucose readings rather than relying on pulse symptoms alone.
What if my weight rises quickly during heart-failure treatment?
A rapid weight increase with ankle swelling or worsening breathlessness may indicate fluid retention. Contact the heart-failure team promptly rather than stopping carvedilol or changing the diuretic on your own.
