Bosentan 125mg is an endothelin-receptor antagonist for pulmonary arterial hypertension. It reduces resistance in the lung circulation and can improve exercise capacity in appropriately selected patients.
How to use
Swallow with water twice daily, in the morning and evening, with or without food. Adult treatment commonly starts at 62.5mg twice daily for four weeks before a planned increase to 125mg twice daily; body weight and tolerability can change that regimen.
How it works
Blocking endothelin receptors limits endothelin-driven constriction and thickening of pulmonary blood vessels.
Side effects
Headache, flushing, ankle swelling, anaemia and increased liver enzymes can occur.
Important information
Liver tests are needed before treatment and usually monthly thereafter. Pregnancy must be excluded and prevented; bosentan can reduce the reliability of hormonal contraception used alone. Pregnancy prevention continues for one month after stopping treatment.

Yellow eyes, dark urine, persistent nausea or right-sided abdominal pain need prompt assessment.
Can the contraceptive pill be the only pregnancy prevention?
No. Bosentan can reduce hormonal contraceptive exposure; the specialist should arrange reliable additional contraception.
Does ankle swelling always mean treatment is working?
No. Fluid retention may be an adverse effect or worsening disease and should be reviewed.
Can I take ciclosporin or glibenclamide with it?
These combinations are contraindicated in major product labels because of interaction and liver risks.
Can I stop if breathlessness improves?
Contact the pulmonary-hypertension team first; withdrawal can lead to clinical deterioration.
Does treatment replace follow-up exercise assessment?
No. Symptoms, exercise tolerance and other PAH measurements are still needed to judge response.
